I suspected o-ring damage but when I pulled the sleeve, I found a 1/4" by 1/2" slash in the side that penetrated the sleeve. The gash looks to be about the width of the cam chain. I ordered a new sleeve but want to know what happened. I can reach inside and feel the chain. It is not broken and seems to clear the sleeve.
